Penatalaksanaan MRI Vertebrae Lumbal Pada Kasus Hernia Nukleus Pulposus (HNP)

Abstract

Hernia nucleus pulposus is the exit of the nucleus pulposus from the intervertebral disc which presses on the spinal cord, causing pain. The purpose of this study was to describe the management of MRI of the lumbar vertebrae in cases of herniated nucleus pulposus. This type of research was a descriptive study conducted in January 2022 at a hospital in the South Jakarta area, involving 3 patients. Data was collected through direct observation and related secondary data collection. The research instrument used was worksheets and documentation tools. The image results were evaluated and strengthened by the expert opinion of the radiologist. The examination used a Philips Ingenia CX 1.5 Tesla MRI machine and a surface coil. The sequences used were Myelo-radial, CorT2W TSE, Sag T1W TSE, Sag T2W TSE, Sag STIR TSE, Tra T2W TSE, and Tra T1W TSE. It was concluded that HNP L-3 to L5-S1 with canalis stenosis, hypertrophy of the ligamentum flavum L4-5 and L5-S1, grade 1 spondylolisthesis L4-5.

ABSTRAK

Hernia nukleus pulposus merupakan keluarnya nukleus pulposus dari diskus intervertebralis yang menekan spinal cord sehingga menyebabkan nyeri. Tujuan penelitian ini mendeskripsikan tata laksana MRI vertebrae lumbal pada kasus hernia nukleus pulposus. Jenis penelitian ini adalah studi deskriptif yang dilaksanakan pada pada bulan Januari 2022 di salah satu Rumah Sakit di wilayah Jakarta Selatan, yang melibatkan 3 pasien. Data dikumpulkan melalui observasi langsung dan pengambilan data sekunder terkait. Instrument penelitian yang digunakan adalah lembar kerja dan alat dokumentasi. Hasil citra dievaluasi dan diperkuat dengan pendapat expertise dokter spesialis radiologi. Pemeriksanaan menggunakan pesawat MRI Philips Ingenia CX 1,5 Tesla serta surface coil. Sequence yang digunakan adalah Myelo-radial, CorT2W TSE, Sag T1W TSE, Sag T2W TSE, Sag STIR TSE, Tra T2W TSE, dan Tra T1W TSE. Disimpulkan bahwa HNP L-3 s/d L5-S1 dengan canalis stenosis, hipertropi ligamentum flavum L4-5 dan L5-S1, spondilolistesis L4-5 grade 1.
